How can I request a Student Certificate and Transcript?
You can request one from the Student Affairs office on the 1st floor of T Block.
Can Someone Else Collect My Documents?
Your documents can only be collected by someone you have authorized through a notary public.
When and Where Can I Obtain My Provisional Graduation Certificate?
First, the College will announce your graduation decision, and the "Provisional Graduation Certificate" will be prepared within 15 days of your graduation date. You can collect your document from the Student Affairs Office (F Block, 1st Floor).
When and How Can I Obtain My Diploma?
Diplomas must be issued within one year of graduation. You can pick up your diploma from the Student Affairs Office (F Block, 1st Floor).

I Lost My Student ID. How Can I Renew It?
You can apply to the Student Affairs Office on the 1st Floor of T Block.
How Can I Change My Address/Phone Number, Etc.?
You can update your information by contacting the Student Affairs Office. Your phone numbers and email addresses must be updated so that academics and administrative staff can reach you in urgent situations.
Is Class Attendance Required?
There is a 70% attendance requirement for classes.

How Long Can I Apply for a Registration Suspension?
Applications for a leave of absence can be made up to the 7th week following the start of classes.
What are the Requirements for a Leave of Absence?
To leave your registration, you must have completed at least one semester of study.
You can apply to the School of Health and Safety Directorate using the leave of absence form in the information and documents section of our website.
Your application will be subject to approval by the School of Health and Safety Board.
Valid excuses for leaving your registration are:
a) The student's temporary travel abroad for educational purposes related to their profession or education, or their temporary assignment to a domestic institution for the same purpose.
b) Military service.
c) Illness, natural disaster, financial difficulties preventing further study, or similar reasons requiring work, the death of a relative such as a mother, father, spouse, child, or sibling, or the absence of another caregiver in the event of their illness.
d) A student who has successfully completed their coursework has had their thesis work interrupted due to reasons beyond their control, such as difficulties obtaining equipment or materials.
How Many Semesters Can I Leave My Registration?
You can defer registration for a maximum of two consecutive semesters.
How to Apply for Registration Cancellation?
You can apply to the Student Affairs Office with a petition for registration cancellation. Your petition is submitted to the School Administrative Board for approval, and the Student Affairs Office will notify you of the outcome, whether positive or negative.
What is Internal Transfer?
You can transfer from the program you are currently enrolled in to another program. You can apply to the Student Affairs Office with an internal transfer form. To qualify for internal transfer, you must have completed at least one semester of study.
How Can I Object to My Grades?
You can appeal your grade by filling out the form displayed by clicking the "Objection" button under the "Exam Results" menu in your Student Information System and submitting it to the Institute Secretariat.
What is a Supplemental Exam and Who Can Take It?
For all courses in which you received a failing grade (including those for which you did not take the final exam), you can take the supplemental exams held on the dates specified in the academic calendar without applying. You can find the schedule for the supplemental exams, as well as the final exam schedules, on your student page. The grade you receive from the make-up exam counts towards your final exam grade. In other words, the final exam is the make-up exam, not the course itself.
